Habilitation & Respite Services, also known as State Services, are provided for DDD (Division of Developmental Disabilities) qualified members.
Habilitation
Habilitation services focus on the direct implementation of Individualized Service Plan (ISP) goals centered predominantly around daily living, basic life skills, improving behavior, and independence in the client’s home and community. Individuals of all ages qualify for this service and is approved by the client’s state assigned Support Coordinator. The Support Coordinator determines the number of authorized hours per week which vary depending on necessity. Parent (or guardian) involvement is essential and required for this service and is not a replacement to childcare. Parents (or guardians) of the client may refer family members, close family friends, and trusted client’s natural supports to HOPE Group to provide this service, even parents (and guardians) can provide Habilitation services.
Respite
Respite services give short-term relief for parents (or guardians) and is provided in the home and community. Individuals of all ages qualify for this service and is approved by the client’s state assigned Support Coordinator. All clients approved for this service are authorized the same number of hours per year. Parents (or guardians) of the client may refer family members, close family friends, and trusted client’s natural supports to HOPE Group to provide this service, although parents (or guardians) are not authorized to provide Respite services.
