Where can I access timesheets, composite schedules, etc. for my payroll processing?
Timesheets are no longer available online. Upon hire, a file containing all internal policy documents was shared with you via email. To request this file again please contact human resources here. If you would like copies physically mailed to you, your can provide that information in your email. Please note it does take up to three business days for requests to be processed.
Where can I get my client’s Habilitation Reports?
When you initially start with your client our Clinical Services Department will email you a copy of the most current typed Habilitation Reports. It is your responsibility to maintain copies of these reports. If you would like copies mailed to you, please contact the office. When we receive new goals we will email them to you as well.
Who is eligible for medical and dental benefits?
Employees may qualify for insurance benefits through HOPE Group if they work 120 hours per month for three consecutive months.
What other benefits are available?
HOPE Group offers 401K is you are 21 years old and have worked for one full year. If you have any questions you may contact our human resources manager here.
How do I know when I am eligible for benefits?
Our Human Resources Department will contact you once you have fulfilled the requirements to obtain benefits.
Can I have a copy of my CPR/FA, Article 9 or Prevention and Support Training?
Copies of the CPR / First Aid certification from HOPE Group can be obtained at a cost of $60 per card. To purchase these cards, please contact our front desk Administrative Assistant. To request employee records, complete the form here. Article 9 and P&S certificates are property of HOPE Group, so we are unable to give those out. You are welcome to call us and ask when you took the class, then you can call DDD and they can verify your certification.

How do I find more clients to work with?
There are several ways to increase your work hours. To request a list of clients in need of providers or to inquire about being matched with a specific client please complete this form.

What if I need to report an incident to HOPE Group outside of Business hours?
In the event of an incident or accident with a client, you will need to contact our staff on call at 480-399-1880. Please keep in mind that this number should only be called outside of business hours. The main line should be used during normal business hours. Once complete, download the incident report form here and submit the form, with your signature via email or fax within 24 hours of the incident.
I need to turn in an incident report but can’t get to the office?
We are required to submit Incident Reports within 24 hours because of this we are able to accept faxed or emailed copies. You are required to also mail us the original Incident Report with in 7 days.
I’ve left a voicemail and/or sent an email to the office and have not heard back. What is the estimated response time for office staff?
Each employee at HOPE Group strives to return calls/emails as quickly as possible, however, we do generally maintain a 2-business day turn around.
How do I obtain employment verification?
We can accept signed written or verbal consent to release employee records to a designated entity. Please call the office for more information.
What is yearly training? Which classes are accepted as a yearly training class?
As a Habilitation provider of HOPE Group, you are required to take a yearly training class in order to expand your knowledge in the field of ABA. The yearly training does not apply to providers only billing Respite. We offer a variety of classes at our Phoenix, Glendale, and Tucson offices on a monthly basis, and also offer webinars for your convenience. Your yearly training class has to be an ABA based class. Do I have to have direct deposit?
To conserve resources, HOPE Group uses a paperless payroll system and therefore all checks will be processed as direct deposit. If you do not have a bank account, we can provide you with a no cost TFG bank card. For more information contact Human Resources.
Why am I receiving encrypted messages?
We are required to keep all client information confidential. Having messages encrypted ensures that their personal information remains within in the HOPE Group server and is no accessible by third party vendors. When you receive an encrypted email follow the directions to either request a temporary one-time code or you can register your current email address. These messages are not accessible through cellular devices unless you download the OWA App. If you cannot open the emails or are having a hard time setting up your email please contact the office.
How do I find out how many hours my client has left?
When you begin providing services, human resources will inform you of the service authorization details. It is your responsibility to track these hours, however should you forget or need additional clarification please contact client services here.
How do I know if my certifications are up to date?
It is your responsibility to ensure all employment requirements are kept up to date, including all certifications and yearly training requirements. HOPE Group does make reasonable attempts to send reminders of these due dates as they arise. However, these are just courtesy notices and are not guaranteed, especially in circumstances in which your contact information has changed. If you are unsure of your certification expiration dates or you need to verify the employment information we have on file, please contact the office.
What happens if I allow any of my certifications to expire?
Maintaining certification is a requirement of all employees. Failure to submit updated certification information by the original certification due date will result in termination. You will be eligible for active employment status once the certifications are up-to-date and all additional trainings are completed.

What if I got married/divorced and my name has changed?
You can easily update your information completing this form. Once processed, new tax forms will be emailed to you. You are required to complete and return these forms within 7 days.
How can I renew my Fingerprint Clearance Card?
If you need to be fingerprinted contact human resources to schedule an appointment. If you are unable to come into the office you can reach out to your local Police station and request for them to fingerprint you. The cost of the card is $67. Because the card is personal property, you are responsible for the cost of the card. We do, however, offer flexible payment options. Once you receive the card from the Department of Public Safety you can upload a front/back copy of the card here, or email a front/back copy to the office here.
I completed First Aid/CPR with an outside agency, how do I submit my cards for proof of certification?
